Depending on your needs, this procedure can be tailored in two ways:

- Upper blepharoplasty: focused on removing the excess skin ‘hood’ that hangs over the mobile eyelid. This excess skin not only makes the eyes look older, but in severe cases can cause a feeling of heaviness and even limit the visual field.
- Lower blepharoplasty: Lower blepharoplasty: aimed at treating under-eye fat bags and pronounced tear troughs. This technique softens the transition between the eyelid and the cheek, permanently eliminating the appearance of chronic fatigue.

A key diagnosis in the first consultation
In your first consultation (free of charge) we assess your anatomy comprehensively to ensure that surgery is the right solution for you. We evaluate four determining factors:
Eyebrow position
We assess whether the excess skin originates from the eyelid or is due to a lowering of the eyebrow. In some cases, the key is to elevate the gaze and not just to remove tissue.
Eyelid ptosis
We determine whether ptosis (weakness of the levator muscle) is present. If the eyelid is drooping, we repair the muscle during surgery to achieve a more open gaze.
Eye health
We perform tests to measure eyelid laxity and the presence of dry eye syndrome, ensuring that the procedure is safe for your vision.
Volume and skin
We analyse skin quality and the origin of under-eye bags or hollows, allowing us to decide whether to remove fat or reposition it to fill dark circles.
Blepharoplasty results and longevity
- Fresh look effect: we achieve an immediate rejuvenation by eliminating signs of tiredness and heaviness. The result is a brighter and more open look, ensuring that your expression and personal features remain intact.
- Recovery and scar maturation: although improvement is visible after 2 to 3 weeks, tissues and scars continue to refine for up to 6 months. Full maturation of the healing process is completed 6 to 12 months after the procedure.

Types of eyelid surgery
Not all eyes age in the same way. While some people notice a loss of firmness in the upper eyelid, others present a change in volume or hollows below the eyes. Techniques associated with lower blepharoplasty

- Fat repositioning: instead of removing all the fat, we reposition it to fill the hollowness of the dark circles, preventing a sunken appearance.
- Canthopexy/canthoplasty: if the eyelid presents laxity, we reinforce the tension of the eyelid, maintaining a harmonious shape and preventing it from pulling downwards.
- Malar lipofilling: in cases of loss of volume in the cheekbone, micro-fat grafting is used to smooth the eyelid-cheek transition.
Surgical approaches and scars
We always choose the least invasive technique to guarantee maximum discretion:

Transconjunctival approach (no external scar): this is performed through the inside of the eyelid. It is ideal for treating under-eye bags when there is no excess skin. The scar is invisible.

Subciliary approach: the incision is made just under the eyelashes to tighten the excess skin. Over time, the scar becomes an imperceptible line that blends in with the edge of the eye.

Volume restoration: fat repositioning and lipofilling
- When is it necessary: for pronounced dark circles, deep tear troughs or flattened cheekbones.
- Why reposition fat: ageing also involves a loss of volume. Simply removing under-eye bags can result in a ‘flat’ or hollow eyelid. Fat repositioning and/or lipofilling restores naturalness.
- What to expect after fat repositioning: part of the transferred fat is reabsorbed. After 3-6 months 60-80% is usually retained. Once integrated, the transferred fat is permanent and will naturally fluctuate with weight gain or loss.

What to expect after blepharoplasty surgery
Understanding each stage of the process and having the reassurance of a safe hospital environment are the keys to making your recovery as satisfying as the result itself.
The recovery process for blepharoplasty is usually quick and with minimal discomfort.

- Swelling and bruising: usually peak in 48-72 hours, and then gradually improve every day. The application of local cold for the first 48 hours is recommended to reduce inflammation.
- Temporary tearing, gritty sensation and slight tightness. More frequent if you suffer from dry eye syndrome.
- Stitches: removed after 5-7 days.
- Screen use: from day 2 - 3 after surgery. It is important to rest your eyes for the first few days.
- Use of make-up: from day 7-10 onwards (skin permitting).
- Use of contact lenses: generally, from the 2nd week onwards.
- Physical activity: walking from the first day. Light cardio after 2-3 weeks. Strength and impact exercise from 4-6 weeks onwards.
- Sun exposure: it is important to wear sunglasses and take appropriate photoprotection measures for 1 month.
In lower blepharoplasty procedures involving fat repositioning or lipofilling, we usually prescribe eye drops, lubricating ointments and gentle massages.

- Common clinical course: during the first few days, bruising, eye irritation, a sensation of dry eye or conjunctival swelling (the white membrane of the eye) may occur. These are transitory effects that disappear with the prescribed medication.
- Skin details: sometimes small keratin cysts (milia) or very slight asymmetries may appear during the healing process.
- Eyelid position: in lower blepharoplasty, there is a minimal risk of the eyelid losing its natural position. To avoid this, we always assess the firmness of the tissue and, if necessary, perform a reinforcement technique (canthopexy) during the same operation.

However, for a comprehensive rejuvenation, surgery can be combined with neuromodulator treatments to soften these lines, achieving a much more rested look.
It depends on the cause. If dark circles appear too dark due to the hollowness or the shadows projected by the under-eye bags, fat repositioning and malar lipofilling can significantly improve the appearance. If the darkness is caused by skin pigmentation (brown or purplish discoloration), there are specific dermatological treatments that can complement the results of surgery.
