Breast implant replacement
Over time, patients with breast implants may need or decide to undergo a new surgery. The reasons may be purely aesthetic (change the size, shape or type of implant) or due to complications such as capsular contracture, rupture or displacement of the implant.
At HC Aesthetic we cover everything from the simplest replacements to complex secondary surgeries, involving the correction of multiple problems: poorly positioned implants, noticeable asymmetries, excess skin, visible scars, alterations of the gland or deformities after previous surgeries.
Our goal is always the same: to restore the shape, symmetry and harmony of the breast using safe and personalised techniques adapted to each case.
When should breast implants be replaced?
At HC Aesthetic, breast implant replacement is always planned in a personalised way, adapting the technique to the anatomy, goals and lifestyle of each patient. In the first consultation (free of charge) we review your medical history and perform a complete assessment, which includes:
Rupture or wear of the implants.
Capsular contracture (hardening and deformity due to fibrosis)
Displacement or malposition (very high, low or laterally positioned implants)
Asymmetry, or a change in implant size, shape or type by personal choice.
Correction of aesthetic sequelae from previous mastopexy, reduction or augmentation procedures.
Need to improve the appearance of previous scars.
Scarring in breast implant replacement surgery
In most cases we use the existing scars so as not to generate new ones. In more complex surgeries, it may be necessary to enlarge or modify them to obtain adequate access and improve the aesthetic outcome. In many cases, this can even improve the appearance of the previous scar.
Breast implant replacement surgery tailored to you
Each case requires an individualised assessment, as the patient’s needs, tissue condition and goals may differ significantly. Therefore, the initial examination is essential to determine the most appropriate approach and to define precisely the type of intervention that will provide the best possible result.
The initial assessment includes:
-
- A detailed clinical and surgical history.
- Thorough physical examination to assess tissue coverage, symmetry and the condition of the existing scars.
- Imaging tests (ultrasound or MRI) if there is suspicion of capsular rupture or contracture.
- Establishing the goal: are we aiming for an aesthetic change, to correct a problem, or both?
Planning according to complexity:
-
- A simple replacement, with well-positioned implants and healthy tissues, is quick and with recovery similar to a primary augmentation.
- A more complex secondary surgery may require reconstructing the implant pocket, fat grafting, mastopexy, or even several procedures in a single operation.

What to expect after breast implant replacement
-
- Hospital stay: usually 24 hours.
- Drains: may be necessary in complex replacements, which are removed in the first days after surgery in the office.
- Discomfort: mild in simple replacements; slightly greater if extensive surgery is performed.
- Return to work: 1-2 weeks for simple replacements; longer for complex surgery.
- Physical activity: avoid strenuous exercise for 4-6 weeks.
In most cases, these complications are resolved with medical treatment or minor surgical refinements.
-
- Haematomas o seromas.
- Temporary alterations in sensation.
- Visible or widened scars.
- Partial persistence of asymmetries.
- Recurrence of capsular contracture.
-
- Aesthetic and functional improvement of the breast.
- Greater symmetry and harmony in the body contour.
- Feeling of renewed security and confidence.
- Final results are visible after 3-6 months, when the scars have softened and the tissue has settled.

Frequently asked questions about breast implant replacements
We understand that undergoing a second operation generates specific doubts. In this section we answer the most common questions about techniques, recovery and safety in breast implant replacement.
Unlike the implants used in previous decades, there is no fixed ‘expiry date’ for today’s cohesive gel implants. They are designed for extremely high durability and resistance. However, at HC Aesthetic we recommend regular check-ups by ultrasound or MRI to ensure their integrity.
Replacement is only considered in three scenarios: if there is a complication (such as rupture or capsular contracture), if the patient wishes to change the volume or if the breast tissue has aged and requires an aesthetic adjustment.
In simple replacement procedures (same location and size), the procedure is similar to the initial surgery.
However, complex secondary surgeries (where the position of the implant needs to be corrected, capsular contracture treated or an associated mastopexy performed) require more planning and technical expertise. In these cases, the surgery may take slightly longer and recovery is usually somewhat slower, always under strict medical supervision at our hospital.
Yes, implant replacement is the ideal time to update the appearance of your breasts according to your current preferences.
Many patients use this procedure to switch from round to anatomical implants (or vice versa), or to adjust the volume to fit their current lifestyle. During the initial assessment, we perform a proportion study to ensure that the new design harmonises perfectly with your overall silhouette.
