Breast lift surgery
Mastopexy is surgery to lift, reshape and restore firmness to sagging breasts (breast ptosis). Over the years, pregnancy, breastfeeding or significant changes in weight, the skin loses elasticity and the breast tissue descends, causing the breast to look hollow at the top and heavier or sagging at the bottom.
The goal of a mastopexy is not only to reposition the breast to a more youthful position, but also to redefine its shape, improve symmetry and, if necessary, restore volume and projection by placing implants.
Unlike other surgeries, mastopexy always involves working on the skin and gland together, which requires strategically planned scars to achieve a stable and long-lasting result. Rather than a “drawback”, these scars are the tool that allows us to achieve the change you are looking for.
When is a breast lift indicated?
No two bodies are the same and there is no single surgical approach that suits all patients. For this reason, planning begins with an initial free consultation, in which the plastic surgeon carries out an exhaustive medical assessment to define the most appropriate technique, ensuring natural results and preserving safety at every stage of the procedure.
Nipple descent below the inframammary fold.
Marked skin flaccidity.
Loss of firmness and projection of the upper pole.
Breast asymmetry associated with ptosis.
Change following pregnancy, breastfeeding or weight loss.
Personalised mastopexy
- Degree of ptosis: we assess whether the breast droop is mild, moderate or severe, a key aspect in determining the surgical technique and the type of scar required.
- Breast volume: we assess whether the existing volume is sufficient or whether mastopexy should be combined with an implant to improve the shape and projection.
- Skin quality: we evaluate the elasticity, skin thickness and the presence of stretch marks, all factors that influence the result and its stability.
- Symmetry: we identify possible differences between the two breasts in order to correct them and achieve a harmonious and natural result.

Scars in mastopexy
In mastopexy, scars are not a side effect: they are the key to achieving a good result. Without scars it is not possible to effectively tighten the skin or reposition the breast tissue and nipple in a stable position.
In reality, even if they are visible at first, well-managed scars fade in comparison to the aesthetic and functional benefit of having a lifted, firm and proportionate breast.
-
- Periareolar: around the areola, for mild ptosis.
- Vertical: around the areola and a vertical line towards the crease, for moderate ptosis.
- Inverted-T: adds a line in the inframammary fold, for severe sagging or marked excess skin.
-
- They are the surgical tool that allows us to reconstruct a breast with improved shape, symmetry and firmness.
- They remove excess skin, preventing the breast from drooping again quickly.
- Over time, most of them fade to the point of being barely noticeable.
-
- Multi-layer closure to reduce tension.
- Silicone dressings and gels from weeks 2-3 onwards.
- Gentle massage and specific creams once the skin has healed sufficiently.
- Strict photoprotection during the first year.
What to expect after mastopexy surgery
Everything you need to know about your recovery, how your results progress over time and possible considerations.
-
- Hospital stay: 24 hours
- Usually, no drains are required.
- Discomfort: well-managed with oral medication.
- Return to work: return to work in 1-2 weeks depending on the nature of the job.
- Physical activity: avoid strenuous exercise for 4-6 weeks.
- Postoperative bra: continuous use during the first month.
While they are rare and most are mild and resolve with medical treatment or minor corrections, the following may occur:
-
- Haematomas or seromas.
- Temporary alterations in sensitivity, especially in the areola and nipple.
- More visible scars than usual (hypertrophic or keloid scars in predisposed patients).
- Minor asymmetries in shape or height.
- Partial necrosis of the areola or nipple (very rare, more frequent in severely sagging breasts or patients who smoke)..
-
- Immediate improvement in the position, shape and firmness of the breast.
- More balanced body contour.
- Final results are visible after 3-6 months, when the scars have softened and the tissue has settled.

Frequently asked questions about mastopexy
We answer the most common questions we are asked in consultation to help you clear all your doubts.
In aesthetic medicine, the removal of excess skin necessarily requires an incision. In cases of very mild sagging (pseudoptosis), minimal incision techniques can be used, but for moderate or severe corrections, scars are essential to achieve a long-lasting and harmonious result.
At HC Aesthetic, we place the scars strategically (periareolar, vertical or inverted-T) and apply advanced healing protocols so that, over time, they are practically imperceptible.
In most surgeries, the connection of the lactiferous ducts to the nipple remains intact, allowing breastfeeding.
However, it is important to note that in surgeries that require extensive reshaping or major tissue resections, the capacity for milk production may be reduced. If you are planning a pregnancy in the short term, our team will advise you on the technique that best preserves glandular function.
The results of mastopexy are long lasting. The breast regains a youthful, firm position that lasts for many years. To prolong this result, it is essential to maintain a stable weight, keep the skin well hydrated and wear an adequate support (bra).
Although the biological ageing process and the effects of gravity continue, operated breasts will always have a much more aesthetic shape and position than if the surgery had not been performed.
